Submit a Rebate - Instructions

To receive your rebate, you must complete and submit a Rebate Form so that it is received by the Claims Administrator no later than August 2, 2023. Your submission must also include (1) a copy of the sales receipt(s) showing (a) the date of sale, (b) the amount paid for the product(s), and (c) the product name and style number, along with (2) a copy of the UPC code(s) from the product packaging.

The sales receipt(s) and UPC code(s) are required for proof of purchase. This rebate may be transferred but will only be valid for one user. The same purchaser name must appear in the Rebate Form and supporting invoices. Purchases from more than one individual may not be combined in support of a claim for a rebate. The Claims Administrator: (i) will not accept claims if the documents submitted appear to be modified or are illegible; (ii) reserves the right to verify any and all information submitted; (iii) shall make all determinations regarding the validity of the information and all such decisions shall be final; (iv) reserves the right to request additional information and documentation, including serial numbers for the products, to support the claims; and (v) is not responsible for lost, late, missing, or illegible rebate submissions, or misdirected submissions. Please retain a copy of your Rebate Form and all documents submitted for your records. If a rebate submission is incomplete or fails to meet any of the rebate terms and conditions, no rebate will be issued. The rebate check must be cashed no later than 30 days from the date of issue.
